You may either fill it out and scan it and email it to Ms. Ali at alichilds4@gmail.com. After which you will recieve an email back confirming I received the information.
Or, you may fill it out and mail it to the school. Please call Miss Ali at (385)-226-1213 and she will give you the address to the school. Please feel free to call for a walk through as well.
